Stellate Ganglion Block (SGB) for patient with PTSD

Presenting Complaint

The patient presented with a 5-year history of severe PTSD and complex PTSD. Symptoms included constant flashbacks, nightmares, hypervigilance, distressing thoughts, ‘being on the edge’ and a feeling that something bad was going to happen. The patient also had a history of depression over the past 5 years.

Previous Investigations/ Treatments

The patient had seen various specialists in the past including psychiatrists, psychologists, and counsellors. He was given a diagnosis of post-traumatic stress disorder (PTSD). He tried various treatments including anti-depressants, CBT, psychotherapy, EMDR, and meditation. These treatments helped him intermittently for short periods.

Treatment from Dr. Krishna at Pain Spa

The patient saw Dr. Krishna at his private clinic at the Willow Surgery in Bristol, desperate for some answers. The patient had researched extensively about the role of SGB in the management of PTSD symptoms. Dr. Krishna discussed the treatment in detail and explained all the possible benefits and associated side effects. The SGB was performed under real-time ultrasound guidance at C6 level with long-acting local anaesthetic. The patient felt immediate benefit following the injection and was discharged home.

Outcome

The patient’s progress was monitored with PCL-5 scores at 1 week and 3- months post-procedure. The PCL-5 scores indicated significant improvement in his PTSD symptoms. The patient has described the treatment as life-changing!

We will continue to follow his progress over the next 6 to 12 months to see if the benefits from the treatment are long-lasting.
