Stellate Ganglion Block (SGB) in PTSD
Presenting Complaint
39 year old presented with a 8-year old history of severe PTSD. Symptoms included repeated disturbing memories, repeated disturbing dreams, feeling very upset when reminded of a stressful experience from the past, avoid thinking about stressful experiences from the past, loss of interest in things he used to enjoy, feeling cut off from other people, sleep problems, feeling irritable and angry outbursts, difficulty with concentration, being super alert, feeling jumpy/ easily startled and physical symptoms like palpitations and sweating.
Previous Investigations/ Treatments
The patient had seen various specialists in the past including psychiatrists, psychologists, and trauma counsellors. He was given a diagnosis of post-traumatic stress disorder (PTSD). He tried various treatments including anti-depressants, CBT, psychotherapy and EMDR. Unfortunately none of these treatments were successful in his case.
Treatment from Dr. Krishna at Pain Spa
The patient saw Dr. Krishna at his private clinic at the Willow Surgery in Bristol, having researched extensively about the role of SGB in the management of PTSD symptoms. Dr. Krishna discussed the treatment in detail and explained all the possible benefits and associated side effects. The SGB was performed under real-time ultrasound guidance at C6 level with a long-acting local anaesthetic. The patient felt immediate benefit and described the feeling as ‘a heavy load off his shoulders’.
Outcome
The patient’s progress was monitored with PCL-5 scores at 1 week and 3- months post-procedure. The PCL-5 scores indicated significant improvement in his PTSD symptoms. The patient came back for a repeat procedure after a few weeks and gained further benefit from it. He managed to go on a month’s holiday and continues to feel relaxed. The patient has subsequently described the treatment as life-changing!
