Background

A 65-year-old female presented with a long-standing history of severe low back pain that significantly impaired her daily life. Prior to treatment, her condition had deteriorated to the point where she was nearly housebound, with limited mobility and worsening quality of life.

Clinical History

The patient had been evaluated by multiple specialists over the years. An MRI scan revealed degenerative changes in both the intervertebral discs and the lumbar facet joints. Despite various consultations, surgical interventions were not considered appropriate by the spinal surgeons. She had previously undergone physiotherapy, but unfortunately, this exacerbated her pain rather than providing relief.

Initial Consultation and Diagnosis

The patient was referred to Dr. Krishna at Pain Spa in Bristol for further management of her chronic low back pain. Based on her clinical history and radiological findings, she was suspected to have facet joint-related pain. Dr. Krishna proposed a diagnostic procedure using lumbar medial branch blocks to confirm the diagnosis. Following a positive response to the diagnostic injections, the patient was deemed suitable for radiofrequency denervation of the lumbar facet joints.

Treatment

Two years ago, the patient underwent bilateral lumbar radiofrequency denervation under fluoroscopy guidance. The procedure was performed by Dr. Krishna, an expert in radiofrequency techniques, and was aimed at ablating the nerves responsible for transmitting pain from the facet joints. The treatment was conducted with precision and no intraoperative complications were reported.

Outcome

The patient reported excellent pain relief following the radiofrequency procedure. With her pain under control, she was able to engage in a structured rehabilitation program, which included weight loss and physical conditioning. At her 6-month and 18-month follow-up reviews, she remained thrilled with the results. Not only was she pain-free at 18 months post-treatment, but her overall function had significantly improved. She was able to resume many of her normal daily activities that had been limited prior to the treatment.

Discussion

Radiofrequency denervation of the lumbar facet joints is a NICE-recommended treatment for managing facet joint-related pain. The procedure, when performed by skilled practitioners, can provide long-lasting pain relief, with results that can last up to two years. The technical challenge of this procedure requires specific training and expertise, as it involves ablating the medial branch nerves that supply the facet joints under image guidance.

Conclusion

This case illustrates the effectiveness of radiofrequency denervation in managing severe lumbar facet joint pain, particularly when conservative treatments such as physiotherapy have failed. Dr. Krishna’s experience and precision in performing this technically demanding procedure played a critical role in the successful outcome. The patient’s significant improvement in both pain and function underscores the importance of considering radiofrequency denervation for patients with similar clinical profiles.
