Successful Treatment of PTSD with SGB injections under Ultrasound
A 36-year-old male with treatment-resistant PTSD experienced life-changing improvement following ultrasound-guided stellate ganglion block (SGB) treatment performed by Dr. Krishna at Pain Spa, Bristol.
Introduction
A 36-year-old gentleman presented with a long-standing history of severe post-traumatic stress disorder (PTSD) secondary to significant psychological trauma from childhood abuse. Despite extensive psychological and psychiatric interventions, including counselling, cognitive behavioural therapy (CBT), and eye movement desensitisation and reprocessing (EMDR), his symptoms persisted.
Case Summary
The patient had been under the care of psychologists, counsellors, and psychiatrists over several years. His symptoms included recurrent intrusive memories and flashbacks, distressing recollections as though the traumatic experience were happening again, and intense physical reactions such as sweating and palpitations when reminded of his trauma. He persistently avoided thoughts and situations associated with his past experiences and felt emotionally detached from others. Additionally, he suffered from chronic insomnia, poor concentration, hypervigilance, and exaggerated startle responses. These symptoms significantly affected his quality of life, relationships, and ability to work.
Treatment Details
Following a detailed consultation, Dr. Krishna discussed the option of stellate ganglion block (SGB) as a potential treatment for refractory PTSD. The procedure was performed under ultrasound guidance, ensuring precision and safety. The right-sided SGB was performed first, followed by the left-sided injection 24 hours later. The patient developed a Horner’s syndrome immediately after each block, indicating a successful sympathetic blockade.
Outcome
The patient reported immediate and remarkable improvement in several of his symptoms, particularly anxiety and the persistent feeling of fear and doom. He described the sensation as though a heavy weight had been lifted off his shoulders. At his six-week follow-up, his PTSD scores had significantly improved, and this benefit persisted at both six and twelve months. Two years later, he underwent repeat SGB injections with similarly excellent results. His overall quality of life improved dramatically, allowing him to rebuild his social connections, maintain a family life, and begin preparations for employment.
The patient has been advised to continue with ongoing psychotherapy as needed to support long-term emotional recovery and resilience.
Discussion / Expert Insight
Stellate ganglion block (SGB) can provide significant relief for patients suffering from severe, treatment-resistant PTSD by modulating the overactive sympathetic nervous system involved in stress responses. This advanced technique requires precision and expertise for safe and effective outcomes. Dr. Krishna is a recognised expert in performing SGB procedures using real-time ultrasound guidance, ensuring optimal accuracy and safety.
